Random crashing while playing

Since my graphic card died a few months ago, I've started getting some trouble while playing bo2 zombies. At some random point in the match, my game just alt+tabs me and I just can't go back to it. When I type "seta r_fullscreen 0" and "vid_restart" on the bootstrap, trying to resuscitate it, it completely crashes and returns me, in the bootstrap, the following:

And after that, a error message pops in the screen returning the crash id of "7c0123d942b6d9e" (don't know if it means something specific about the error or just a random id to my dmp file. Whatever the case, mentioning that too)

CPU: Intel Core i7 3770;
RAM: 2x 4gb Ddr3;
HDD: "Generic" 1Tb;
Monitor: 1920x1080 television but play in 1024x768 (because apparently it matters in other games crash reports);
Supply: 500W

Halberdier to be honest, I just un installed and re installed the complete plutonium launcher, It worked so well before but some update is wreaking havoc. Hope this helps
