Xbox controller not working

I was playing zombies and everything was working fine when all of a sudden my controller could only walk and look around. No shooting, no grenades, crouching, sprinting etc. Also related but maybe not related is that sometimes when my controller is plugged in plutonium cannot even run world at war and crashes. Is their a fix for any of this?

Maybe you have Steam opened in the background
Try fully exiting it -
I have done that and I have also disabled steam controller input completely and have steam turned off

now it is back to not even starting
