Zombie Server bugs

This is for the devs, maybe they can look into these bugs.

Bugs:

After a while of the server being up, and all players leave, or sometimes when the map changes after a game, you get the EXE_ERROR_MODIFYING_SESSION and this makes it so no players can join the server. Restarting the server or changing the map may fix this.

Sometimes after a while of the server being up you may also get failed to connect to host I have only had this big twice, and I have no idea what causes it. But once you either change the map in console or restart the server, it goes away.
