Myself and a friend have also experienced this issue twice:
First time: Friend was the host, map was Shi no Numa, friend crashed on the transition between round 15 and 16
Second time: I was the host, map was Der Riese, I crashed on the transition between round 15 and 16.
This is a fresh install of Black Ops 1 + Multiplayer from Steam, I've uploaded my console.log + crash dump below:
