you need a IWI to DDS converter and follow these steps
TL;DR of this is: find the texture in your BlackOps/Main folder open the IWD file (with Winrar or 7Zip)
find the texture you wanna edit, convert the IWI file to a PNG (using the converter) edit it, convert it to a black ops 1 iwi file and then put it in your '%localappdata%/plutonium/storage/T5/images' folder (which you'll probably need to make the images file yourself)

NotGelus maybe try using an outline/cut-out of the image you are using, black will stay black and any other color will be blood red
