Try setting your headset as default output device in the windows sound settings.
Also make sure to plug in your headset before starting the game.

You shouldn't use DMZ. Make sure you're actually in the game (not sitting in the pre-game lobby), you can start the match by typing "xpartygo" into the console.
Also it's ip:port (e.g. "connect 143.212.42.21:4976").
If it still doesn't work post a screenshot of your port forwarding settings.
