ILoveToEatBalls Don't play the campaign via plutonium, I had a similar problem with the first mission when the tank thing (forgot the name) appears, after a few seconds it pops up "Your actions caused your team to die.".
Only happens when I play via plutonium.

extract the iwi files to gamelocation/t6r/data/images
if you don't have the folders just create them.

Not sure, but you can put the plutonium launcher on the taskbar and launch it via that.
