System > Sound > Device properties > Additional device properties > Advanced tab, and disable "allow applications to take exclusive control of this device".

Launch plutonium.exe as admin so it can update the client files and don't launch it directly through %localappdata%/Plutonium/bin.

Think it's caused if you don't have your frames capped (unlimited/high FPS causes abnormalities in the cod games). Try capping your frames to anything lesser than 300 with the com_maxfps dvar in console.

Is your FPS uncapped? That's known to cause this, try capping your FPS below 200 with the com_maxfps dvar.
