ktfjulien I can probably add ya and do some testing tomorrow. It's very possible that the deny all rule could have something to do with it which would necessitate a port forward.
We can test out joining each other's games and seeing what traffic gets captured. Also im pretty sure opnsense has some logging we can check

Servers can ban you if they want to

Most likely you got votekicked and it's automatic or it's one of these trash servers where they ban dozens of people for "cheating" every day the moment you go above 1 K/D ratio

Either contact them or play on another server or make your own

bizzzzz if you want to keep using voicemeeter, you should try and disable other devices in order not to have too many audio devices present in cod, which breaks the mouse and keyboard and gamepad.

Pretty sure this is due to playing on servers with mods, whenever i joined a modded game and leave it resets my stats but i could be wrong but if im not just play servers that use no mods!
