Plutonium doesn't support campaign, buy the game if u want to play the campaign

He has to join with the Plutonium overlay when you are in a game (not lobby)

Sorry did a mistake I got this problem with every maps that have a scene where you select Game difficult etc… before start game

Pvt. Gabriel Hosting a custom game with or without a mod is the same thing, you start a solo game and he joins. We don't use the World at War profile system. As long as you launch the game through the Plutonium launcher and as long as you're connected to the internet you are online, ignore what the game says.
The only thing that changes is that you both have to install the mod/map and have the exact same files with the same size/name etc

Hjbpenguin mods/Clinic Of Evil is one map. You don't put files here only mod folders. Then inside mods/Clinic Of Evil you have COE files.
There is no usermaps folder on World at War.

So I got this all to work finally so I wanted to leave an update for anyone with the same troubles in the future. I could not get the steam copy to work with map changes and custom maps for the life of me. I wonder if this is because of the T4 mod or something (no clue). But I used the t4 game files for the server hosting and my steam copy for playing the games. Using this combination worked just like the instructions in the Plutonium provided guides suggest.
